JEA Scholarship Donation (Type amount of your donation.)	$50 $20 $10 $5 other: JEA awards an annual scholarship of $1,000.00 to a graduating JPS student pursuing a career in education. Please consider donating to help us sustain this award. (Note: Please include this amount in the payment to JEA. If you are paying by Payroll Deduct, we will deduct this amount with your dues.)
Payment Method*	Payroll deduction is for TEACHER members only (by ND law). This method deducts the dues amounts for JEA/NDU directly from your paycheck., ACH is an option to have dues deducted directly from your checking account., Dues Credit Trust - select this only if you are already signed up and turn your local dues check in to your Building Rep., Pay by check - Write a check for your total dues + contributions and turn in to your Building Rep.
Courtesy Policy (Voluntary)	I agree to the policy as described below for the period of August 22, 2022 through August 15, 2023. Member obligations and benefits shall include: 1. Upon the death of a member or spouse of a member of this policy, $10 will be paid to the JEA treasurer within 30 days of the occurrence. The treasurer will then forward the sum to the beneficiary, as designated by that member, within an additional 10 days. NOTE: You, as a participant, will be contacted before any money is collected. NO MONEY IS DUE AT SIGN-UP. * This sign-up must be done yearly, you must specify a beneficiary!

Final Agreement*	I understand that checking below verifies that I agree that my membership continues from year to year unless I revoke this authorization in writing prior to September 30 of the current school year or in subsequent years. If for any reason my employment is terminated prior to the end of any contract, the dues for which I am obligated shall be pro-rated by the number of months I am employed for the contract year. I also understand that the dues amounts for the NDU and JEA will be determined annually. Furthermore, I have submitted truthful and accurate information.
